Do I need a referral to begin services?

Yes! We will need a referral for speech therapy from your primary care provider faxed to (970) 541-0357 to begin services.

Do you offer screenings to determine if my child is eligible for Speech Therapy services?

Yes! We offer an initial phone screening with one of our therapists to discuss your concerns or a 15-minute in-clinic screening to determine if ongoing therapy is appropriate for your child.

If my child is receiving services in school, can they still be seen in a private clinic?

Yes! We are eager and happy to collaborate with your child’s school-based team as well as provide additional services to compliment your child’s IEP/504 and community-based needs.

Do you take Medicaid?

Yes! We gladly accept Medicaid and CHP+.

I don’t have health insurance. Can my child receive services?

Yes! We have a private pay rate as well as a sliding scale based on income.

CAN I REQUEST MEDICAL INFORMATION AND/OR A HOME EXERCISE PROGRAM?

Yes! If you need medical records faxed to you or your doctor, a copy of a home exercise program, or other patient information, please email Irma at irma.chacon@summitspeechteam.com

Please include the following in the email:

  • Your name

  • Your child’s name

  • Child’s date of birth

  • Treating therapist

  • Information being requested

Irma will follow up with you regarding any additional information needed.
